Hope I'm not too late to get involved with this.
.htaccess:
In dreamweaver, I just start a new html page and write my commands. when it comes to saving it, I type .htaccess in the filename field and set it to "any/all files" in the drop down box. it then gets saved as .htaccess
For the headers, I found that in some browsers, it didnt work as intended. So after searching around for a while, i came up with this:
Seems to work in all browsers and forces a reload of any page it is on.
It doesnt need to be at the top of any file, but it does need to be before any output

Well I have sorted out the problem, but I'm always open to learning new things & learning new ways of doing things. So thanks, I'll be sure to install Dreamweaver next time & give it a try!
