As I predicted, every one of the 83 pages listed by Google for The Affiliate Guide Book website have now been cached.

Maybe I'm no longer under penalty. After Yahoo's last update, 2 of my pages are now cached, and slurp has been spidering almost every page on my site.
A few weeks ago, I created a Yahoo txt sitemap and submitted it to Yahoo from my Yahoo account as recommended.

Glad to hear you're crawling out. YaH is notoriously slow for re-inclusions, lifting penalties etc....

I'm still not even sure if 2 pages cached is even necessarily an indication of penalty removal or not. I have not found any references to this in all my research. It seems to be a mystery to everybody except for those who work for Yahoo. I don't think Matt Cutts would have an answer, as he is a Google employee. I will likely have to wait until Yahoo's next update of my site links to see any new changes. I'm still not receiving any traffic from Yahoo.
